    symptoms, track progress over time, and build a plan that is practical and sustainable.

  • When something changes, it helps to have a trusted place to start. We can evaluate common concerns,

    recommend next steps, and guide you to the appropriate level of care.

  • Medications should be reviewed, understood, and simplified when possible. We can help with medication

    review, refills when clinically appropriate, and coordinating updates after outside visits.

  • When needed, we can arrange labs, review results with you clearly, and coordinate referrals or additional

    evaluation. With your consent, we can also communicate with family members and caregivers.

  • We support proactive care planning, safety concerns, and ongoing follow-up that helps maintain

    independence and quality of life. Caregivers are welcome and encouraged to be part of the plan.
